Preheat oven to 375°. In a large bowl, combine all ingredients except tomato / Teriyaki sauce mixture and reserved onions. Gently stir until everything is distributed evenly. Place mixture in slightly over-sized baking dish and shape into a loaf. Cover with about 1/2 of tomato / Teriyaki sauce mixture and the reserved onions. Bake for about 1 hour or until temperature of loaf reaches about 165 degrees. Let it rest, covered, for 20 minutes and serve with remaining tomato / Teriyaki sauce mixture.
NOTE: A wooden spatula works well to mix ingredients gently, going around and around the large dish until combined. This will also keep the mixture from getting too compacted.
